By filling out this form, you give permission for UUSS to supervise and offer activities and snacks/water to your child in your absence which will not extend beyond 4:00 PM on Saturday, Dec. 14, 2024. You agree that staff and background-checked volunteers (2+ unrelated adults) will make every effort to protect the safety and well being of your child in your absence. You also agree that you will return to retrieve your child in an emergency or if it is determined that their behavior is not compatible with the safety of others. Thank you!

We are happy to host your child/youth during the Holiday Crafternoon-Parents'/Guardians' Afternoon Out. This event is presented free of charge to UUSS Pledging Member Families and Newcomer Families considering membership. UUSS will provide supervised play, craft activities, holiday music and snacks between 1:00 and 4:00 PM on Saturday, December 14 in Room 11 and 12 (enter at far gate near the playground).
